Linking People - A 'Best Practice' Service

Well done to Age Concern Solihull's Linking People team!

Linking People, Age Concern Solihull's gateway and referrals service, has been named as a Best Practice Service by the Health Inequalities National Support Team.  The Project, funded by supporting people at Solihull Care Trust will be taken into other areas as an example of good working practice with a view to replication for the benefit of older people, their families and carers.

Linking People assesses the low level needs of any person over the age of 50 living in Solihull.  It then contacts other agencies and organisations on behalf of the individual and makes referrals and discusses issues.  An outreach worker visits those most vulnerable to offer support.  This project aims to enable older people to remain living independently in their own homes for as long as they wish and covers issues such as benefits, aids and adaptations, energy, caring responsibilities, falls, smoke alarms, home safety checks, winter warmth, accommodation, social and leisure activities and much more.
